FIBROMYALGIA – THE ASSISTANCE FILE

Why the Collective of Associations Fibro’Actions has developed a file support for the care? Because he was asked by the council of the order of doctors to lobby doctors to change things about Fibromyalgia. The Collective of Associations has set up an evolutionary tool for patients with fibromyalgia to facilitate their relationship with doctors and administrations.

This accompaniment file is intended to remain in the possession of the patient . It aims to facilitate the exchange with health professionals and administrations. He may also support the steps taken by the patient, such as a request for ALD, an appointment with a medical adviser CPAM, an application for disability, AAH etc. It also facilitates the communication of the patient to the doctor.

Principles of use:

Describe your pains and their impact  – Page 3: This section is intended to help the patient pre-fill it before the consultation in order to describe precisely what he feels and the impact on his daily life. The doctor will be able to take full measure of the pains and their consequences.

Describe your sleep  – Page 5: The patient should pre-fill this section before the consultation in order to describe precisely what he feels and the impact on his daily life. The doctor will be able to take full measure of the problems related to sleep and their consequences.

Complementary examinations  – Page 9:  FRAMEWORK RESERVED FOR DOCTORS This section aims to facilitate the work of the doctor and allow him to prescribe this battery of minimal tests to rule out certain pathologies. The doctor will then have to complete the conclusions of the examinations practiced and collect the information requested later, in order to be able to move more easily towards a diagnosis.

Patient Orientation  – Page 11:  FRAMEWORK RESERVED FOR PHYSICIANS  The purpose of this section is to facilitate the work of the physician and to be able to see the patient correctly at the appropriate health professionals at a glance. It also allows the patient to visualize the various health professionals attached to his file.

Medication Prescriptions  – Page 12:  FRAMEWORK RESERVED FOR PHYSICIANS This section aims to reflect the history of the prescriptions given to the patient and to be able to judge the positive and undesirable effect of each molecule that has been prescribed. This is also intended to guide a diagnosis.

Felt Patient / Medication Treatments  – Page 14: This section aims at the patient can at any time (even at home), describe his feelings, the positive effects or side effects of the molecules that are prescribed, the goal being of course to communicate it to your doctor but also to leave trace of the fact that you support, or not, such or such molecule which can justify the stop of a treatment for example.

Psychological follow-up & behavioral and cognitive therapy  – Page 15: FRAMEWORK RESERVED FOR DOCTORS This section aims to have a follow-up and an evolution of the patient’s condition visible through this file.

ALD . – Page 19:  FRAMEWORK RESERVED FOR PHYSICIANS  The purpose of this section is to trace the ALD requests made for the patient, and their acceptance or not by the Caisse.

MDPH steps . – Page 20: This section is intended to help the patient to approach the MDPH if necessary.
